Home and Community Habilitation
This service is provided in home and community settings to assist individuals in acquiring, maintaining, and improving the skills necessary to live in the community, to live more independently, and to participate meaningfully in community life. Companion Services
This service is provided to individuals living in private homes for the purpose of providing supervision or assistance that is designed to ensure the individuals health, safety, and welfare or to perform activities of daily living for the individual. Homemaker/Chore
This service assists individuals with maintaining their primary private home. Services provided include cleaning, laundry, meal preparation and other general household care. Behavior Support
Behavior support provides specialized interventions to assist an individual with increasing adaptive behavior while replacing or modifying a challenging behavior of a disruptive or destructive nature. Using data from the Functional Behavior Assessment, a Behavior Support Plan is created which is followed by the team to see positive changes in an individual’s behavior. The desired goal is to enhance the individual’s overall well-being and participation in daily life activities.
